Documents wit the application

Hello Folks,

On another note, I have so far collected the following documents.

1. Employment letters from my last 2 employers (last 5 years) which state my position, duration of employment, job description and my responsibilities, certify that all the work was in English and my salary.
(is it necessary to mention the salary? they are not asking for it)
2. My MA certificate from german university, a letter from the univ saying that the entire MA was in English, transcripts for all the semesters
3. My BA certificate from osmania university and transcript
4. Bank statement for the last 6 months
5. Tax returns for 2009 to show that I really earned and that is not temporary money in the bank
6. Copy of my passport which shows the visa stamp of 2 years in Germany
7. Birth Certificate

What else is needed?
Is a cover letter needed?
Is it necessary to submit a CV which shows all my 12+ years of experience? In this case, do I need to submit reference letters from all my employers?

I have other Diplomas as well. Is it necessary to submit those as well?
